Hi Pstoric,

Please open Outlook in Safe Mode and check how it works.
To do that, press the 'Windows + R' key on the keyboard and type in 'Outlook /safe' and hit OK in the Run open box.
Note: There is a space between Outlook and /.
If application safe mode works, try disabling the add-ins associated with Outlook and check.
Here are the steps:
1. Open Outlook.
2. Click on File and then click on options.
3. In the Options Window click on Add-ins.
4. Select Com Add-ins in the Manage list in the Add-ins section and click Go. Now uncheck the all the add-ins and click OK.

If the issue persists in safe mode, I'd like to confirm the following information:
1. How many email accounts are configured in Outlook? What is the email account type, Exchange, IMAP or POP?
2. Are you able to sign in to Outlook Web App with the same credential?
3. Does the issue occur if you configure the account in another workstation or you configure other normal account in the affected workstation?

Thanks,
Franky

Yea the issue occurs in safe mode as well. There is only one account/profile configured in Outlook and it's an office 365 account.

We can successfully log into OWA via outlook.office365.com

hi pstoric,

thanks for your updates.

you can try running microsoft office 365 support and recovery assistant (sara). if the issue persists, please confirm the following information for further troubleshooting:
1. test your office 365 account's "outlook autodiscover" via this link: testconnectivity.microsoft.com/. click "office 365"> "microsoft exchange outlook connection test"> "outlook autodiscover".
2. do an outlook email auto-configuration test:
hold "ctrl" on your keyboard> right click the "outlook" shortcut in the task bar (generally, the task bar is in the lower right corner of your computer)> choose "test email auto configuration". it takes about one minute to generate the log.

i have sent you a private message to collect the two test results for analysis. you may access the link below to check the pm.

Hi Pstoric,

Now, I can see your test results. After reviewing the results, I have found that you are using "mutual authentication" in the "Email auto-configuration" test result. Given the situation, I'd like to confirm whether you are using Office 365 Multi-Factor Authentication for your Office 365 account. If so, try disabling "muti-factor authentication" and check the result. If you are not familiar with this, you can ask for help from your organization's admin.
More information can be found here: Office 365 Muti-Factor Authentication

Also, you can try clearing the Office 365 account's credential in "Credential manager". Here are the detailed steps:

a.    Click Start, click Control Panel, and then click Credential Manager. Note: If View by is set to Category, click User Accounts first, and then click Credential Manager.
b.    Locate the set of credentials that has Outlook in the name.
c.    Click the name to expand the set of credentials, and then click Remove from Vault.
d.    Repeat step c for any additional sets of credentials that have the word Outlook in the name.

If the issue persists, I'd like to confirm the following information for further troubleshooting:
1.    Does the issue persist in another PC?
2.    Is there any improvement if you try a different network environment (out of your organization)?
3.    How many users have the same issue in your organization?

Any update is appreciated.

Regards,
Alan

I'm the admin for the organization.

Prior to submitting this I've cleared all the credentials in the credential manager but that did not seem to help. Creating a new Outlook profile on a different PC seemed to work ok. May open an Office 365 ticket to get this resolved a bit quicker as I am only that the office every Monday for a few hours.

Hi Pstoric,

Regarding "May open an Office 365 ticket to get this resolved a bit quicker as I am only that the office every Monday for a few hours", I'd like to collect some tenant information to help you open a service request in order to help you resolve the issue faster. If you don't the tenant information, please involve the global admin of your organization into this thread to provide the tennat information.

To protect your privacy, I've sent you a private message to collect the tenant information. Please follow the link below to respond to it:

It's strange. I just did a complete removal of Office 2013, cleared the credentials again in the credential manager and re-installed Office 2016 from her Office 365 portal but it's still prompting for the credentials when opening Outlook. If I click cancel it disappears and Outlook connects to Exchange without and problems but it shouldn't be prompting. I can see the entry in the credential manager as well.
